The following links will take you to web pages of some common City application forms. Completed forms must be submitted, with payment, to the Permit Application Center, 555 Liberty Street SE, Room 320, Salem, OR 97301.

HTML Forms

After clicking on the links, print the form page on your printer by clicking on the PRINT option on your browser. Forms are designed for 8.5 x 11 inch paper with 1-inch margins.
